WHAT IS BREAST AUGMENTATION?

Breast augmentation, also known as breast enlargement, uses implants to create fuller-looking breasts patients desire. With different breast implant shapes, sizes, and profiles available for use, the process can be tailored to the unique body proportions and desired results of each patient. The procedure can also be further customized in terms of implant placement and incision location to ensure every woman achieves results that are natural, comfortable, and beautiful.

Dr. Mashhadian has created a unique recovery system known as Recovery EXL, which helps patients recuperate faster after surgery and accelerate the healing process. CANDIDATES FOR BREAST AUGMENTATION

Optimal candidates for breast augmentation are women with one or more of the following concerns:  Breasts are smaller than they would like, asymmetrical breasts, 
changes to breasts as a result of pregnancy, nursing the aging process

Women seeking this surgery should be in relatively good health and non-smokers. Smokers will be asked to quit a few weeks before and after surgery to avoid potential complications. At your initial consultation, Dr. Mashhadian will evaluate you and take into account your concerns to determine whether breast augmentation will be a safe and effective option for you.

OPTIONS IN BREAST IMPLANTS

When you decide to move forward with breast augmentation, you will have various decisions to make in preparation for your procedure. One of the first will be the type of implant you want to use, which might include:

SALINE IMPLANTS

These feature a silicone shell that is filled with saline after they are placed in the breast pocket, allowing for greater precision in the augmentation process. They are suitable for younger patients, as the FDA has approved them for women age 18 and over. They also tend to be less expensive. However, there are drawbacks to saline implants as well. If a saline implant ruptures, the results will quickly become apparent when the outer appearance of the breast also changes.

SILICONE IMPLANTS

Silicone implants also have a silicone shell, but come pre-filled with a silicone gel inside. These implants are available in a wide range of sizes to accommodate many different body proportions. They also offer various shapes and profiles, so the implant that will provide you with the most natural, proportionate result can be selected. Silicone implants are approved for women age 21 and over. While they require a longer incision in most cases, they tend to look and feel more like natural breast tissue in most cases and ruptures will not be immediately apparent.

IDEAL IMPLANTS

For women who want the natural look and feel of silicone implants with the safety of saline implants, the only choice is Ideal Implants. The silicone shell of these implants is made of four layers, called baffles, while the interior is filled with saline. The saline is infused into two separate chambers within the implant, so in the event of a rupture, only one chamber is likely compromised. This alerts the patient to the rupture—which the body will safely absorb—with a less urgent need to schedule the repair and replacement surgery. The baffles also give the implant the feel and movement of a silicone implant for a more natural, satisfactory result.

Also known as form stable implants, this relatively new breast implant type features a more durable form and texture that is similar to gummy bear candies. While they have characteristics that are attractive to many women, including fewer ruptures and lower incidences of rippling, these implants are on the firmer side.

The good news is that this variety of implant options allows us to tailor your procedure to your needs based on the look and feel you desire. Dr. Mashhadian will go through each option with you to ensure you choose the implant type that will fit most closely with your personal preferences.

SIZE AND PROFILE

No matter which type of implants you choose, you will also need to select the shape and profile that works best for you. Implants come in both round and teardrop (anatomical) shapes. Round implants offer more upper breast pole fullness, which may be attractive to some women and unattractive to others. Anatomical implants feature a natural breast slope, which may not provide sufficient fullness for some women.

The profile of the implant refers to how much it projects from the breast wall. Some women prefer a higher profile to achieve a more dramatic result, while others may want a low to moderate profile to produce a more natural-looking enhancement. Dr. Mashhadian will listen carefully to the effect you hope to obtain, so he can help choose the best size and profile for you.

IMPLANT PLACEMENT

There are two basic placement options for your breast implants: Subglandular and Submuscular.

SUBGLANDULAR

The implant is placed between the breast tissue and the pectoralis (chest) muscle. This implant placement is often not recommended for patients with less soft-tissue coverage because the edges of the implant are more likely to be apparent on the outside of the breast. External rippling may also be a more significant concern, and capsular contracture rates may be somewhat higher with this placement option.

SUBMUSCULAR

The implant is placed beneath the pectoralis muscle, with either a portion of the muscle covering the implant or total muscle coverage. This technique offers the best coverage of the implants, which is beneficial for women with a limited amount of natural breast tissue. However, individuals who do a lot of strength training or weight lifting may not like the distortion of the chest muscle during exercise with this placement option.

INCISION LOCATION

Dr. Mashhadian uses four different types of incisions during the breast augmentation procedures he performs:

  • Periareolar incision – made around the areola, the colored area surrounding the nipple
  • Inframammary incision – extends horizontally underneath the breast in the breast crease
  • Transaxillary incision – created in the armpit to reduce visible scarring on the breast
  • Transumbilical (TUBA) incision – made in the belly button to ensure no visible scarring after the procedure
  • Trans-abdominoplasty incision – much like TUBA, but for patients undergoing a full-body Mommy Makeover transformation. The implants are placed during a tummy tuck procedure to avoid extra incisions and post-operative scarring

No matter which incision is used for your procedure, Dr. Mashhadian will carefully place them in locations where they will be least noticeable after surgery. In addition, Dr. Mashhadian’s use of the Keller funnel for his implant procedures allows for smaller incisions than traditional insertion techniques require. Women are usually pleased to discover that scarring after breast augmentation is safely hidden under clothing or bathing suits so that they can flaunt their new breasts with confidence.

Before your surgery, Dr. Mashhadian will walk you through the decision-making process, guiding you through each choice to ensure your procedure is ultimately targeted to your specific needs and desired outcome. By the time your surgery date arrives, you should feel confident of your choices and ready for the surgery and recovery process that lies ahead.

HOW IT WORKS

Breast augmentation surgery takes about one to three hours at our cosmetic surgery facility in Beverly Hills. Dr. Mashhadian’s breast augmentations are outpatient procedures, which means you should be able to return home the same day as your surgery. Patients may receive either local anesthesia with sedation or general anesthesia, based on other medical conditions they might have and if they are comfortable sleeping through the procedure. Like the other decisions surrounding breast augmentation, Dr. Mashhadian will support you through each choice to ensure your procedure is precisely geared to your concerns, body proportions and aesthetic goals.

During the surgery, Dr. Mashhadian will make the incisions you previously discussed to insert the implants into the breast pocket or underneath the muscle. He uses state-of-the-art techniques like the Keller funnel for this process to produce the best possible results using the least invasive process. The Keller Funnel is a device used to place a silicone implant into the breast using a smaller incision and creating less trauma to surrounding tissue. It also reduces the possibility of infection entering the breast pocket during the surgery. Once the implants are in place, he will suture the incisions closed, using tiny stitches with the goal of minimizing visible scarring after surgery.

Advanced techniques allow for a faster recovery than patients might experience with less experienced surgeons. Dr. Mashhadian’s unique Recovery EXL system reduces discomfort after surgery and allows you to recuperate without the need for narcotic pain medication. Dr. Mashhadian will also prescribe antibiotics to prevent infection. Bruising and swelling are common following breast augmentation surgery, but instructions are given to minimize these side effects as much as possible. Patients are required to wear a surgical bra that provides additional support to the breasts as they are healing. Strenuous activity and direct stimulation of the breasts are discouraged for four to six weeks. Dr. Mashhadian will advise you when it is safe to resume all your regular activities based on your recovery progress.
